RESIDENT CARE SERVICES RATES

 

Case Management for Assisted Living Program - $44/hour

Ø     Medication management (inventory, ordering, documentation, physician verification)

Ø     Physician, resident and family coordination/communication

Ø     Service plan implementation and monitoring

Ø     Discharge/transfer coordination

Ø     Ordering of durable medical equipment

Ø     Outpatient therapy coordination

Ø     Lab work coordination

Ø     Assessments (respiratory, mental, hydration, nutrition, general)

 

Medication Administration - $34/hour or $8.50/15 minutes

Ø     Administering and setting up medications

Ø     Inoculations/Shots

 

Nursing Services - $44/hour or $11.00/15 minutes

Ø     Wound Care

Ø     Tube Feeding

Ø     Lab work coordination

Ø     Physician appointment coordination

Ø     Vital signs monitoring

Ø     Glucose monitoring

 

Assistance in Daily Living - $23.00/hour or $5.75/15 minutes

Ø     Bathing and Showering assistance

Ø     Personal Hygiene Assistance

Ø     Medication Reminder

Ø     Incontinent Care

Ø     Turning and Positioning

Ø     Feeding Assistance

Ø     Meal Set-up, Preparation or Delivery

Ø     Catheter Care

Ø     Simple Dressing Changes

Ø     Errands/Grocery Shopping

 

Companion Services - $19 per hour

Ø     Services are provided by our licensed C.N.A. staff – 4 hour minimum

 

COORDINATED SERVICES

Billed directly to resident by Physician, lab, pharmacy or healthcare provider

Ø     Monthly Podiatry Clinic

Ø     Weekly Physician Clinic

Ø     DLS Lab Availability for Blood Draws

Ø     Prescriptions filled and delivered

Ø     Nutrition Consultations

Ø     Physical Therapy Services

Ø     And all other coordinated services

Personalized service plans will be completed and reviewed with residents and/or their representatives.  Each resident will be assessed on an individual basis and recommended services need to be agreed upon prior to implementation.

 Residents have the right to refuse service or service plans.
